Communication Preferences

For routine matters, the preferred methods of communication are the Spruce app or phone. The Spruce App offers secure messaging and other options.

Unencrypted email is not secure and should not be used for sending protected health information. Dr Horn's email address is [email protected].

Messages are typically returned within 1 business day.

After Hours Phone Calls

When the office is closed, you may call the main telephone number to leave a message for Dr. Horn which he will return during the next business day, or for urgent matters, call 888-406-4560 to reach Dr. Horn directly. In an emergency, you should call 911 or go to the nearest emergency room.

Parking

On-site parking is available in the front of the building.  Upon arriving in the lot, patients should scan the QR code via smartphone.  Patients will then enter their license plate and credit card information.  Once registered, at future visits, upon scanning the QR code, patients will automatically be billed for the parking session.
